  • A copy of the Explanation of Benefits (EOB) from the first insurer
  • A copy of the original receipt from the service provider
  • A completed claim form if required by the second insurer

Make sure the receipt includes all required info: name, license number, service date, clinic address, and provider signature.

  • Plan A (your employer) covers 80% → $80 reimbursed
